If you have installed the Visual Basic 6 Runtime Files an have MSCOMM32.OCX installed I would recommend just downloading the executable ModemCheck.exe 32K.
If not, download the full package ModChkInst.exe 1.43 Meg.

To uninstall the program go to Windows Control Panel --> Add/Remove Programs --> ModemCheck --> Add/Remove
